Kindergarten and new 1st Graders - Need to submit a physical and dental exam form. These exam forms must be dated after September 1 of the previous year.
2nd and 6th Graders - Need to submit a dental exam by the first day of 3rd and 7th grades. These exam forms must be dated after September 1 of the previous year.
5th Graders - Need to submit a physical exam form by the first day of 6th grade. These exam forms must be dated after September 1 of the previous year.
New Students in all grades are required to have a current physical and dental exam form.
