PHYSICAL EXAMINATIONS

Physicals are due upon entry into school and 6th grade. Physicals done a year before scheduled due date will be accepted. Physical forms will be handed out in 5th grade and you can also obtain a form here by clicking on the link provided.

DENTAL EXAMINATIONS

Dental Examinations are required upon entry, 3rd grade and 7th grade. Private Dental Exams done a year before they are required will be accepted. Dental Forms for 3rd grade will be sent home in 2nd grade and dental forms for 7th grade will be sent home in 6th grade or the form is available by clicking on the link provided.
